Maternity call record

The NHS Scotland Perinatal Telehealth Project is progressing the development, evaluation
and implementation of an auditable clinical template and decision support tool which will
assist midwives in making clinically appropriate telehealth assessment and decision making
regarding women in their care e.g. whether a woman is in labour, timing of admission or
requirement for a home visit.

collated comments from midwives using forms:

* Having been part of the telsis audit initial findings were that it did not alter midwife decision making - are links being made to this audit.
* not enough space to write
* makes conversation with woman stilted, unable to use your own judgement to guide conversation
* too much information to fill in not always relevant to conversation not enough space to write
* sheets appear cluttered unable to see information easily
* very time consuming concentrating on filling boxes rather than listening to the woman
* not practical in this area as we can have calls one after the other and don't always have access to a computer
* no area for medical history
* may be that it is set out for call centre use and not midwives who are working clinically

I would like to add a comment with regard to the reverse side of call record.

As I work in a community midwife unit we often require to advise women to either call or travel to main obstetric unit. I feel in call out come this should be added. It says attend unit for assessment. obstetric unit needs to be added or which unit/hospital.

Hope this is valid?

Nowhere to record presentation of fetus,

Far to much writing

No place for time contractions commenced or SROM

Does not flow as a history it jumps about over the page

Far to time consuming,spaces are too small. Important details don't stand out.

So busy filling form you forget to ask about things, A/N history, Breech/Cephalic etc

We have used telephone triage now for a considerable time approx 6yrs in our maternity unit. We have been used to a particular form but welcome the change for a national format.
The main concern is that although the form does contain all information required, the conversational flow is not as easy to use as our previous form.I am sure this can be remedied easily.

Hi It has been interesting to use a different call record. We at the Triage Departmetn at Striling Royal feel that there is alot of unnecessary information requested and that there is no logic to the information requested. The form does not follow the flow or normal questioning. We found the form too fussy and complicated. It does not allow the clinical midwife to use her judgement with the use of clinical guidelines. We did however like the call back page on the back - very useful and tree saving! Susan Kennedy

The final report is awaiting sign off by the National Maternity Group.

I will keep you posted regarding date of pubication/next steps.
