I have some Health Visitor forms as used in Tayside, covering ante-natal, post-natal, 8 weeks-15 months, and ‘cause for concern’.
I’d like to put them here as draft templates just to get things going. (We are at the end of the feasibility study phase).
Any comments? I’ll add them over the next week, unless you tell me not to :)
Hi Val and all – are we talking about single record for acute care? I don’t know of any – none in Paeds journal although there was a good summary of issues around the integrated care record system (Anderson Dec 2005). Some community services starting to use integrated health/social records around Common Assessment Framework – I guess that would be a useful template??

I am still struggling with plan to develop a single child record…I notice Anne Casey has joined the discussion group which is great. Hope alls well Anne.
Wonder if you know of areas where a single record is in place. We have made some contact with Manchester but cannot find much else.

OK, I’ve put in the ‘Child assessment, 8 weeks to 15 months’ template submitted by NHS Tayside, and as used in the Health Visitor systems there.
You’ll also see a number of other templates, but these are all components used in the main one. In each template you’ll see a bit called ‘used by’ or ‘used in’ to link these all together.
We are testing out the idea of developing ‘building blocks’ which can be used in any number of templates. That way we can get the components sorted out and you don’t have to keep inventing wheels in your systems.
One of the components is a very simple wee item on continence, so I’ve put that in the continence group. This way we get the components owned/maintained by the relevant experts. If they update it, your template will also be updated.
So any comments put into that continence bit will be emailed to members of both groups.
So that’s the big idea, and I’d be very interested in your thoughts on if/how this might work.
