Comments for Falls Assessment

    I work in general practice and have an interest in anticipatory care. I know there are good examples of falls assessments and medication reviews carried out on housebound and elderly. I would like to find out more about what tools are out there and what circumstances they are used, hence this messege to form a group. I know nothing about this area and am keen to get an “off the shelf” tool which could be part of an assessment package. What have you all got?

    Glasgow has an integrated multi-disciplinary approach to falls and osteoporosis, encompassing both primary and secondary care. The primary care element is the Home Falls Prevention Programme (Tel: 0141 427 8311).     I am currently project managing a nurs/midwif documentation review in my hospital.I am eager to learn more about clinical templates and as falls in our hospital make up a high level of incidents that get reported maybe it would be a interesting one to be involved with.

    Hi Anne

    Thanks for your interest. There are people active in falls prevention taking an interest in this group, but so far no one has taken the plunge and contributed some example assessments.

    I’m hoping that might change soon :)

    In the meantime, if anyone has example forms used locally, just send them to me – d.hoy@gcal.ac.uk and we can put them in the ‘resource’ section of this group.

    Take a look in a few other groups to see how this has been used, eg continence or alcohol.
